Write your own record
Use these fields in your own notes. Include fictional or role-relevant information only. This page does not store your notes or create a staff case.
- Subject and current situation. What scene, request, or decision does this record concern?
- Source and established facts. What was observed, what was reported, and what is still uncertain?
- Applicable agreement or rule. Which current expectation guides the decision?
- Action and reason. What was actually done, by whom, and why?
- Status and next owner. Is the task pending, accepted, paused, or complete? Who owns the next step?
- Correction or review point. What changed, who needs the update, and when will unresolved work be reviewed?
Before sharing
Read the record as if you were joining the next shift. Remove assumptions presented as facts, unrelated personal details, and claims of completion without evidence. Confirm that the receiving participant understands the current task. A record that looks formal still needs to be accurate.
Program-specific review criteria
- Use the correct authority and published criteria.
- Separate established facts from claims and uncertainty.
- Explain a proportionate decision and its consequences.
- Assign follow-up and communicate the current outcome.
